This policy explains what information this website (TampaTMJ.com) collects, why we collect it, who helps us handle it, and the choices you have. It covers the website only.
Who we are
Tampa TMJ, P.A. is the practice of Dr. Doraida Leon, DMD, located at 3911 West Waters Avenue, Suite 4, Tampa, FL 33614 (opens in a new tab). In this policy, "we," "us," and "the practice" mean Tampa TMJ, P.A.
The website form is not for medical records
Our contact form is meant for short messages, such as a question or a request for a visit. Please do not include medical history, detailed symptoms, medications, ID numbers, or other health details. The form does not accept file attachments.
If you need to send records or forms, please call the office or email us. Regular email may not be encrypted.
If you are a patient, or become one, the health information you share as part of your care is kept under the practice's patient records practices, not under this website policy.
Please do not use the form for anything urgent. Call us at (813) 772-4563. In an emergency, call 911.
What we collect
When you send the contact form, we collect:
- your name
- your email address, your phone number, or both (we ask for at least one so we can reply)
- your message
- your preferred language, English or Spanish (the form preselects the language of the page)
- the page you sent the form from, and the date and time you sent it
- the result of the automated spam check
When you visit any page:
- Our hosting provider receives the technical information that every website receives, such as your IP address, browser type, and the page you asked for. It uses this to deliver the page and to protect the site from misuse.
- If you allow Analytics, anonymous counts are recorded: visits, and taps to call, to send an email, to get directions or to load the map (see "Cookies and similar tools" below).
- Your cookie choices are recorded (see the same section).
We do not ask for health information through this website, and we ask you not to send it.
How we use this information
- To read your message and reply by phone or email, in the language you prefer.
- To keep a dependable record of every message, so a message is not lost if an email fails to arrive.
- To keep spam and automated abuse out of the form.
- To understand, in total and without identifying anyone, which pages visitors find useful and how they reach us (taps to call, email, get directions or load the map), if you allow Analytics.
- To keep a record of the cookie choices you made.
We do not sell your information. We do not use it for advertising, and the website does not use advertising pixels or advertising cookies. If that ever changes, we will update this policy first and ask for your consent through the cookie banner.
How your message travels
- On the Contact page, Cloudflare Turnstile runs a short automatic check in the background to tell people apart from automated programs. It may start as soon as the page opens, and it runs no later than when you begin to fill in the form. It receives technical information such as your IP address and browser details, and it stores a small security item in your browser (see our Cookie Policy). When you press send, the result of that check goes with your message.
- Your message is saved in a secure database kept for the practice. Access is limited to the practice and to the people who maintain the website on its behalf.
- A copy is emailed to the practice, to info@TampaTMJ.com and drleon@tampatmj.com, through Resend, an email delivery service.
- The practice replies to you directly, by email or by phone.
If neither our database nor our email service receives your message, the page tells you so and shows our phone number, so you are never left thinking a message arrived when it did not.
Service providers
We use a small number of companies to run the website. Each one receives only what it needs for its task and handles information under its own terms and privacy policy.

Maps and outside links
The map on our Contact page does not load until you click "Load map." Until you do, nothing about your visit is sent to Google. Once you load the map, Google's own privacy policy applies to what Google collects.
Links to our social media pages, such as instagram.com/TampaTMJ, take you to those platforms, and their own policies apply there.
Cookies and similar tools
This website uses as few cookies as it can. On your first visit, a banner lets you choose "Accept all," "Reject all," or "Customize." Only strictly necessary items run before you choose, such as remembering your choice, your language and your display settings, and protecting the form. You can change your mind at any time with the "Cookie preferences" link at the bottom of every page. Our Cookie Policy lists each cookie, what it does, and how long it lasts.
How long we keep information
- Form messages stay in the website database for 24 months and are then deleted from it. The copy that arrives in the practice's inbox is handled under the practice's own record practices.
- If you become a patient, information you later share as part of your care is kept under the practice's patient records practices, not under this website policy.
- Consent records are kept for 24 months.
- Our providers' own logs are kept for limited periods under their own terms.
How we protect information
Every page uses an encrypted connection (HTTPS). The database cannot be read from the public website; only the site's own server can write to it, and only authorized people can view it. The form has spam protection and does not accept files.
No website or email system can be made completely secure, so we cannot promise that information sent over the internet will never be intercepted or misused. This is one more reason we ask you not to send health details through the form.
Your choices
- You can always call us at (813) 772-4563 instead of using the form.
- You can reject analytics and change your cookie choices at any time.
- You can ask us what information from the website form we hold about you, ask us to correct it, or ask us to delete it, using the contact details below. We may need to confirm it is you before we act. We may not be able to delete information we need to keep, for example if it has become part of a patient record.
Children
This website is meant for adults. If you are under 18, please ask a parent or guardian to contact us for you.
Where information is processed
Our providers process information in the United States and, for some of them, in other countries where they operate.
Changes to this policy
We may update this policy when the website changes, for example if we add a new tool. The effective date and version at the top will change. If a change affects cookies, the banner may ask for your choices again.
